<div class="detail"> <h4 class="h4_o">长春市中医院东部疫情防控相关设备采购</h4> 发布时间：2021-02-07 14:39信息来源：<label id="platformName">长春市公共资源交易中心(新)</label><a target="_blank" href="http://www.ccggzy.com.cn">原文链接地址</a> <div id="mycontent"> <div class="detail_content">一、项目基本情况 项目编号：HAGJ-HW-2***05 项目名称：长春市中医院东部疫情防控相关设备采购 采购方式：竞争性磋商 资金来源：自筹资金 面向企业类型：非专门面向中小企业 服务地点：长春市中医院 采购需求：医疗设备，详见招标文件服务范围及要求 采购预算：***万元 合同履行期限：签订合同之日起7天之内供货 质量标准：符合国家及行业合格标准 本项目不接受联合体投标 二、申请人的资格要求： 1.满足《中华人民共和国政府采购法》第二十二条及《中华人民共和国政府采购法实施条例》第十七条规定的条件； 2.本项目的特定资格要求： （一）投标供应商为生产企业的，从事第一类医疗器械生产的，应取得食品药品监督管理部门颁发的《第一类医疗器械生产备案凭证》；从事第二类、第三类医疗器械生产的，应取得食品药品监督管理部门颁发的《医疗器械生产许可证》或在有效期内的《医疗器械生产企业许可证》。 （二）投标供应商为经营企业的，从事第二类医疗器械经营的，应取得食品药品监督管理部门颁发的《第二类医疗器械经营备案凭证》或有效期内的《医疗器械经营企业许可证》；从事第三类医疗器械经营的，应取得食品药品监督管理部门颁发的《医疗器械经营许可证》或有效期内的《医疗器械经营企业许可证》。 （三）投标产品若纳入中华人民共和国医疗器械监督管理的，第一类医疗器械须具有备案凭证，第二、三类则应取得监督管理部门颁发的相应的《中华人民共和国医疗器械注册证》及《医疗器械产品注册登记表》，若已办理两证合一则只需提供《医疗器械注册证》。 3.近三年（2017-2019年度）经会计师事务所审计的财务审计报告或财务报表（新成立的企业以成立之后的年份为准）； 4.近六个月任意三个月的依法缴纳税收及社保的相关材料； 5.近三年内无行贿犯罪档案记录证明，提供投标人在中国裁判文书网上查询的无犯罪记录查询结果网页截图； 6.对于近期列入失信被执行人、重大税收违法案件当事人名单、政府采购严重违法失信行为记录名单的投标人，拒绝参与本项目采购活动；【查询渠道：“信用中国”网站（www.creditchina.gov.cn ）或中国政府采购网】（提供未列入严重违法失信企业名单（黑名单）信息的公示证明（打印页加盖投标单位鲜章）； 7.单位负责人为同一人或者存在直接控股、管理关系的不同供应商，不得参加同一合同项下的政府采购活动； 8.拒绝列入政府取消投标期间的企业或个人投标。 三、获取采购文件 时间：2021年<span style="background-position: 0% 0%;
