<p>3.本项目的特定资格要求：（1）在中华人民共和国注册的供应商、具备有效的营业执照（提供三证合一的营业执照复印件加盖公章)。（2）具有良好的商业信誉和健全的财务会计制度；（提供2020年度的财务报表）。（3）具有依法缴纳社会保障资金的良好记录（提供2020年至今任意1个月依法缴纳社会保障资金的证明，复印件加盖公章）； （4）具有依法缴纳税收的良好记录（需提供2020年至今任意1个月依法缴纳税收的证明,复印件加盖公章）；（5）参加政府采购活动前三年内（成立不足三年的从成立之日起算），在经营活动中没有重大违法记录；（提供无重大违法记录声明书，加盖公章）。（6）在“信用中国”网站（www.creditchina.gov.cn）没有列入重大税收违法案件当事人名单、政府采购严重违法失信名单，在中国执行信息公开网（http://zxgk.court.gov.cn/shixin/）没有列入失信被执行人和在中国政府采购网（www.ccgp.gov.cn）没有列入政府采购严重违法失信行为记录名单的投标人。（7）购买本项目招标文件并缴纳投标保证金。（8）本项目供应商资格要求的特定资格要求：A包：提供医疗器械经营许可证及药品经营许可证；B包：生产厂家的需提供食品生产许可证；非生产厂家提供食品经营许可证；根据相关法律法规文件可不提供商品经营许可证的，提供相关文件（复印件加盖公章）；C包：生产厂家的需提供食品生产许可证；非生产厂家提供食品经营许可证；根据相关法律法规文件可不提供商品经营许可证的，提供相关文件（复印件加盖公章）；D包：/；E包：生产厂家的需提供食品生产许可证；非生产厂家提供食品经营许可证；根据相关法律法规文件可不提供商品经营许可证的，提供相关文件（复印件加盖公章）。</p>
