style="font-family:宋体">未被列入失信被执行人、重大税收违法案件当事人名单和政府采购严重违法失信行为记录名单的投标人</span><span style=";font-family:宋体">；</span></p><p style="margin-left: 32px;line-height: 29px"><span style=";font-family:宋体">②</span><span style=";font-family:宋体">投标人为电梯设备制造单位或销售代理单位；</span><span style="font-size:15px;font-family:宋体">供应商为销售代理单位的，必须具有品牌制造商对本项目的授权书；</span></p><p style="margin-left: 32px;line-height: 29px"><span style=";font-family:宋体">③</span><span style=";font-family:宋体">电梯制造单位应具有特种设备（电梯）生产许可证（如销售代理单位投标，须提供所投电梯制造单位的特种设备（电梯）生产许可证）；</span></p><p style="margin-left: 32px;line-height: 29px"><span style=";font-family:宋体">④具有特种设备（电梯）安装改造维修许可证（若特种设备（电梯）生产许可证中已含安装、修理、改造等内容，即特种设备（电梯）生产许可证和特种设备（电梯）安装改造维修许可证二证合一单位，无须再提供此证）；</span></p><p style="margin-left: 宋体">法定代表人的被授权委托人必须是投标单位职工。需在投标响应文件商务部分内提供由社保机构出具的该授权代表的社保证明，社保证明出具时间不得早于投标截止时间前2个月（1.如该授权代表为离退休返聘人员的，投标响应文件内需提供退休证明及单位聘用证明；2.如由第三方代理社保事项的，则需提供加盖投标人公章的委托代理协议复印件）。</span></p><p style="text-indent:
