class="MsoNormal" align="justify" style="text-indent:21.0000pt;text-align:justify;"> 5、质量要求：工程质量要求达到国家施工验收规范合格标准。 </p> <p class="MsoNormal"> 四、<b>供应商</b><b>条件</b><b>：</b><b></b> </p> <p class="MsoNormal" style="margin-left:0pt;text-indent:21pt;"> 1、符合《政府采购法》第二十二条规定； </p> <p class="MsoNormal" style="margin-left:0pt;text-indent:21pt;"> 2、供应商必须具有工商行政主管部门核发的有效法人营业执照，并具备国家和省级有关行业主管部门颁发的建筑工程施工总承包三级及以上资质，需具有建设行政主管部门核发的有效安全生产许可证、企业基本账户银行开户许可证，并在人员、设备、资金等方面具有承担本工程施工的能力； </p> <p class="MsoNormal" style="margin-left:0pt;text-indent:21pt;"> 3、项目经理资格：具备房建专业二级及以上注册建造师资格（提供注册证书）和有效的安全生产考核合格证书（B证）、职称证（工程类中级及以上职称），<span>需提供近</span>6个月<span>（</span>2018年5月至2019年1月任意6个月）本单位缴纳的个人社保证明网页打印件并标识出网址以供查询，且承诺未担任其他在施建设工程项目经理； </p> <p class="MsoNormal" style="text-indent:21pt;"> 4、技术负责人及相关人员：技术负责人应具备工程类专业中级及以上职称，项目管理人员（施工员、质量员、材料员、安全员、预算员或造价员）需提供有效的资格证书或岗位证书、职称证（工程类中级及以上职称）<span>，安全员还需具有有效的安全生产考核合格证书（</span>C证），所有人员需提供近6个月<span>（</span>2018年5月至2019年1月任意6个月）本单位缴纳的个人社保证明网页打印件并标识出网址以供查询； </p> <p class="MsoNormal" style="text-indent:21.0000pt;"> 5、供应商应在“湖北省建筑市场监督与诚信一体化工作平台”进行企业基本信息和人员信息登记（提供报送企业基本信息及拟投入本项目的项目经理、技术负责人及施工员、质量员、材料员、安全员的网页版证明材料）; </p> <p class="MsoNormal" style="text-indent:21pt;"> 6、财务要求：近两年财务状况良好,无亏损，须提2016、2017年度经会计师事务所或审计机构审计的财务会计报表，包括资产负债表、现金流量表、利润表和财务情况说明书； </p> <p class="MsoNormal" style="text-indent:21pt;"> 7、业绩要求：近三年(自磋商响应文件递交截止之日前36个月，以合同签订时间为准）已完工的类似工程施工业绩，需提供施工合同、中标通知书、竣工验收证明书（或竣工验收备案证）及网上下载版中标公示及链接等证明材料； </p> <p class="MsoNormal" style="text-indent:21pt;"> 8、信誉要求：近三年（2016年1月1日至磋商响应文件递交截止之日）内没有骗取中标和严重违约及重大工程质量问题，没有处于被责令停业，投标资格被取消，财产被接管、冻结、破产状态；通过“信用中国”网站（www.creditchina.gov.cn）、中国政府采购网（www.ccgp.gov.cn）查询，供应商未被列入失信被执行人、重大税收违法案件当事人名单、政府采购严重违法失信行为记录名单； <br> 9、其他要求：供应商有假借资质，伪造证件等违规违法行为的,一经发现取消其投标资格；已经中标的,取消其中标资格；已经签订施工合同的,合同无效（提供相应的承诺书）； </p> <p class="MsoNormal" style="margin-left:0pt;text-indent:21pt;"> 10、本项目不接受联合体报名。 </p> <p class="MsoNormal">
