<style id="fixTableStyle" type="text/css">th,td {border:1px solid #DDD;padding: 5px 10px;}</style> <meta charset="utf-***"/><div id="template-center-mark"><p style="white-space: normal;"><samp class="bookmark-item uuid-16239976599***2 code-projectContent editDisable addContent multi-line-text-input-box-cls readonly" style="font-family: inherit;">温州医科大学附属眼视光医院，根据《中华人民共和国政府采购法》有关规定，现就温州医科大学附属眼视光医院机房精密空调维保服务进行公开询价采购，欢迎具备独立法人资格的相关单位前来参加。</samp></p><p class="p2" style="white-space: normal;"><samp style="font-family: inherit" class="bookmark-item uuid-16239976599***2 code-projectContent editDisable addContent multi-line-text-input-box-cls readonly">一、采购项目编号：HQEYE-202202<br/>二、项目名称：机房精密空调维保服务<br/>三、服务地点：温州市鹿城区学院西路270号眼视光医院机房<br/>四、预算金额：***万（供应商总报价不得超过预算价，否则否决其投标）<br/>五、服务期限：1年<br/>六、报价须知：报价截止日期：2022年3月1日下午15:30为止。本次报价供应商在符合资格条件下，报价最低者为本次成交单位。<br/>七、项目概况：<br/>1.精密机房空调位于温州医科大学附属眼视光医院内，负责计算机房的温度湿度的调节，全年24小时满载不间断运行。服务方式：本项目为全包托管维保服务，即供应商通过巡检、保养、修理等各种技术手段保证精密空调正常运行，并承担维护过程中所有的备件、材料、耗材和工具。<br/>2. 付款结算：根据先提供服务后支付原则，半年结算一次，供应商每次申请付款前必须提供本季度制作清单记录并向采购方开具相应金额的、符合国家规定的增值税专用/普通发票。<br/>八、供应商资格要求：<br/>1.必须具有有效的营业执照和独立承担民事责任能力的法人（经营范围需具有精密空调维修及服务）。 <br/>2.供应商需要提供2019年以来类似维保精密空调的维保合同，每份合同的设备清单内需包含世图兹、阿尔西、艾默生中至少一个品牌，并需要提供至少两份维保合同（需附上合同或成交通知书，复印件需加盖公章）；<br/>3.供应商信用良好，近三年内没有受过行政处罚，符合法律、法规规定的其它条件。<br/>九、服务要求：<br/>1.总体要求：<br/>1）供应商负责本次承包的所有设备的故障处理和备件更换。<br/>2）供应商须定期（每个季度一次）为采购方提供全面的设备巡检，对设备做全面健康检查和保养，提前发现设备隐患，提交巡检报告和优化建议。采购方需要临时增加设备检查、健康评估时，供应商应及时响应采购方的要求。<br/>3）供应商须在维护完毕后向采购方提交维修报告。对存在的问题提出解决方案，对使用上的问题提出合理性建议，以保证采购方设备的正常运行。<br/>4）其他技术咨询服务。<br/>2.主要技术及质量要求<br/>1）供应商负责对所有维护设备的检查、维护、检修工作，保证设备正常运行。<br/>2）供应商准备维护范围内所有设备的备品、备件和材料，所有的备品、备件、材料必须为满足设备安全稳定运行的合格产品（如采购方需要需提供质量合格证）。<br/>3）供应商对所维护设备进行巡检（含外机牢固性能测试），发现问题及时处理，并向采购方提供正式的巡检报告。 <br/>4）供应商每季度为精密空调更换过滤网（全新），采购方另有要求的按采购方要求执行。<br/>5）供应商每年冬季为精密空调更换加湿桶1次。<br/>6）供应商有责任对维检的设备及与之相关、但不属于维护范围的，在运行中存在问题、隐患的地方，向采购方报告。<br/>7）供应商维护人员实行24小时工作制，对采购方提出的维护要求应在60分钟内给予现场响应，如超时每次处罚1000元/次； <br/>***）严重故障（停机，不能制冷等），供应商要在1小时内到达故障现场处理故障，并在到达后2小时内解决故障。如不能按时解决，每次处罚2000元/次，超过3次以上采购方有权取消合同。<br/>9）供应商维护人员在工作期间应遵守采购方相应的管理标准、规程、技术，规范作业。<br/>10）采购方发包给供应商的项目不准转包或再分包它方。<br/>11）供应商每次巡检、健康检查后应在个工作日内出具巡检（评估）报告；供应商每年出具维护总结报告；供应商每次处理故障完成后，若采购方有要求，供应商需在个工作日内提交故障处理记录。<br/>12）供应商在合同签订后十个工作日内向采购方提供设备维护方案、计划，并签署安全协议等入场文件。</samp></p><p class="p2" style="white-space: normal;"><span style="text-indent: 15px;">十、设备参数：</span></p><table cellspacing="0" cellpadding="0" width="569" style="width:100%;border-collapse:collapse;"><tbody><tr style=";height:42px" class="firstRow"><td width="1***5" valign="top" style="border: 1px solid windowtext;
