style="margin-right:-15.7500pt;text-indent:20.4500pt;line-height:150%;">欢迎符合条件的投标单位前来报名。请各投标单位提供以下资格要求的证明文件的复印件，且须加盖投标单位印章后方为有效，原件带到招标代理机构备审查后退还，以原件为准。如有伪造或虚报，则招标代理机构有权取消该单位的报名资格。</p><p style="margin-right:-15.7500pt;text-indent:23.0000pt;line-height:150%;"><span style="letter-spacing:0.6500pt;">2、</span>在昆山市注册登记的社工机构，且具有良好的社会信誉；</p><p style="margin-right:-15.7500pt;text-indent:20.4500pt;line-height:150%;">3、执行民间非盈利组织会计制度，有规范的财务管理制度；</p><p style="margin-right:-15.7500pt;text-indent:20.4500pt;line-height:150%;">4、有依法缴纳税收和社会保障资金的良好记录；</p><p style="margin-right:-15.7500pt;text-indent:20.4500pt;line-height:150%;">5、参加本次政府采购活动前三年内，在经营活动中没有重大违法记录；</p><p style="margin-right:-15.7500pt;text-indent:20.4500pt;line-height:150%;">6、如非法人到场报名的，须提供有效的法人授权委托书原件、法人身份证复印件、被授权人身份证复印件（授权委托人必须为本单位员工且须提供投标单位为其缴纳开标前近三个月的社保证明材料）。</p><p style="margin-right:-15.7500pt;text-indent:20.4500pt;line-height:150%;">7、根据评审时“信用中国”网站的信息，对列入失信被执行人、重大税收违法案件当事人名单、采购严重违法失信行为记录名单的供应商，拒绝其参与采购活动。（提供对信用信息查询记录打印件加盖公章）</p><p style="margin-right:-15.7500pt;text-indent:20.4500pt;line-height:150%;">（1）、投标单位要充分考虑知识产权问题，应保证采购单位免除且承担由于投标单位在其本国使用该项目所需的材料和设计或其任何一部分时而引起第三方提出的侵犯专利权、商标权或工业设计权的起诉、行动、行政程序索赔、请求等，以及采购单位为此而产生的损失和损害、费用和支出（包括律师费）。</p><p style="text-indent:20.75pt;line-height:150%;">（2）、投标人负责人为同一人或者存在直接控股、管理关系的不同供应商，不得参加同一合同下的采购活动；</p><p
