0;">供应商需具有独立的法人资格，有履行完成本项目能力，经营范围与所投内容相符，并具备以下条件：①具有独立承担民事责任能力的法人或其他组织，提供合法有效的统一社会信用代码的营业执照（含年度报告书）；开户许可证或基本存款账户信息证明；事业法人应提供事业单位法人证；其他组织应提供合法证明文件；自然人提供身份证；②企业介绍信，法定代表人授权书原件、被授权人身份证原件及法人、被授权人身份证复印件（法定代表人参加谈判时，只需提供法定代表人身份证原件）；③营业执照经营范围应包含城市生活垃圾经营性服务或垃圾清运；④税收缴纳证明：提供依法缴纳税收或完税证明（开标前六个月中任意三个月）；⑤财务状况报告：须提供2019—2021年任意一年度具有财务审计资质的单位出具的财务报告（成立时间至提交文件截止时间不足一年的可提供成立后任意时段的资产负债表、或开标前三个月内银行出具的资信证明，或财政部门认可的政府采购专业担保机构出具的投标担保函）；⑥参加政府活动近三年内，在经营活动中无重大违法记录的书面声明；⑦类似项目业绩：提供2019年至今类似项目业绩贰份及以上；⑧供应商不得为“信用中国”（www.creditchina.gov.cn)中列入失信被执行人和重大税收违法案件当事人名单的供应商，不得为中国政府采购网（www.ccgp.gov.cn）政府采购严重违法失信行为名单；（提供查询结果网页截图并加盖供应商公章，查询日期为从谈判文件发售之日起至谈判截止日前）⑨本项目不接受联合体投标。</p>
