font-size: 16pt">1、本工程采用赣建字【2021】11号文件《关于全省房屋建筑和市政基础设施工程招标投标试行运用建筑施工企业信用评级结果的通知》中的报价承诺法及标准示范格式文本（江西省公共资源交易网网上下载相关文件、电子化评标）。相关的其它补充要求会在网上及时通知，请各投标人密切留意，否则，因此造成的后果由投标人自负。 2、拟投入本项目的建造师（不含临时）、技术负责人、八大员应为与本企业建立了劳动关系的员工，且不得为公务员及本单位以外的事业单位人员（是指在编在岗且是国家财政全额拨款或部分拨款单位工作人员），并提供本企业在本工程开标当月前连续六个月（不含开标当月，社保缴至上月）以上且尚未到期的劳动合同和缴纳的社会基本养老保险证明材料原件。 3、外埠来赣施工单位在参加开标会时须根据《关于省外进赣建设工程企业实行信息登记管理的通知》，进行信息登记入录且审核通过，并在“住建云”上查询到相关信息的企业（即本项目所拟派建造师、八大员等相关人员须与“住建云”单项工程投标登记中一致，须在投标时附上查询网址步骤供评标专家查询，否则，视为投标人自动放弃投标资格），方可参加本工程投标； 4、按法【2016】285号文规定，本工程不接受在全国信用信息共享平台或信用中国网公布的失信被执行人企业投标。本工程不接受6个月内各级建设行政主管部门网上通报存在建市【2007】9号文中“施工单位不良行为记录认定标准”即有以下行为的企业参加投标（企业及项目经理有弄虚作假行为、有串通投标行为、有被限制参加投标行为、有违法行为，有拖欠农民工资行为，有发生重大安全事故的企业，以处罚文件下发之日起计）。投标人三年内未发生任何重大责任安全事故。 5、中标企业一律不得转包本工程，所有管理人员和重要施工岗位人员必须为本企业员工，发现有违反本规定行为，招标人可以取消该中标人的中标资格或中止合同且投标保证金或履约保证金一律不予退回，并将违规行为报行政主管部门。 6、中标人应按饶府办字[2011]20文件规定缴纳农民工工资保障金。 7、本项目施工现场考勤执行《上饶市建筑工程施工现场关键岗位人员刷脸考勤管理办法（试行）》（饶建发【2018】30号）规定； 8、本项目不接受联合体投标； 9、所有潜在的投标人，须严格遵守中华人民共和国、江西省和上饶市等相关招标投标法律、法规，若相关条款有冲突之处，以最新法律、法规和规定为准。 10、本项目执行饶府发【2019】13号文件《上饶市工程建设领域不良行为记录和黑名单制度管理暂行办法的通知》，具体如下：投标单位有下列行为之一的，列入不良行为记录：（1）、投标单位违反规定提供虚假证件、虚假工程业绩的；（2）、投标人扰乱开标、评标秩序造成不良影响的；（3）、累计一年内出现无效标二次及以上的；（4）、恶意诬告、无理取闹投诉、捏造事实或者提供虚假材料上访投诉的； 11、本项目采用"不见面开标" 系统开标。各投标企业的法定代表授权委托代理人无需到开标现场参与本项目开标活动。但委托代理人必须是投标企业本单位职工，并且能够热练的操作新点系统，因业务不熟悉而导致的一切后果由投标企业自行承担。 12、本项目招标控制价为：77764644.34元，招标要约价为：74161094.57元。 13、业绩要求：投标人需提供一个开标当月前近60个月内（不含开标本月度，以竣工验收备案表或专业工程的竣工验收报告的时间为准）已竣工完成的单项合同金额在7700万元及以上的市政工程业绩或4万吨/日及以上的污水处理厂工程业绩（达到80%即可）。同一工程必须提供3种以上（含3种）能证明投标企业施工经历的有关证明材料原件[竣工验收备案表或专业工程的竣工验收报告（必备），施工合同、经备案的中标通知书或直接发包告知书、施工图纸、施工许可证等，但不含文字证明材料]，投标人提供 2009 年 1 月 1 日以后江西省内工程项目中标业绩的，其业绩应当在“江西住建云——住建大数据——项目数据——（检索拟查询业绩名称）——项目活动——招投标或施工许可”或者在“江西住建云——住建大数据——中标数据（从原中标信息管理系统业绩中迁移出）”可以查询到的数据信息业绩，提供外省项目业绩的，应当在外省省级住房和城乡建设行政主管部门或者省级建设工程招标投标监督机构官方网站上公示并可查询；投标企业提供的企业业绩在所要求的网站不可查询的视为未提供企业业绩。投标人在资格审查送审文件中应提供业绩查询路径（业绩网上查询结果必须与所提供材料原件一致，文字证明材料无效）。 14、未尽事宜详见资审文件和招标文件。</span> </td> </tr> <tr> <td align="center" colspan="6"> <span style="font-family: 黑体;
