style="text-indent: 0;">（1）供应商为具有独立承担民事责任能力的法人或其他组织或自然人，并提供法人或者其他组织合法有效的营业执照等证明文件，事业单位应提供事业单位法人证书，自然人应提供身份证明文件；（2）供应商应授权合法的人员参加磋商全过程，其中法定代表人直接参加磋商的，须出具法人身份证原件及复印件，并与营业执照上信息一致。法定代表人授权他人参加磋商的，须出具法定代表人授权书及被授权人身份证原件及复印件；（3）供应商须具备建设行政主管部门颁发的工程监理综合资质或房屋建筑工程监理丙级及以上资质；？？？？（4）供应商拟派项目总监需具备房屋建筑工程专业国家注册监理工程师职业资格，在本单位注册且无在监工程；（5）提供2020年度或2021年度经审计的财务报告，或磋商前三个月内公司基本存款账户开户银行出具的资信证明（以上两种形式的资料提供任何一种即可）；（6）提供2021年1月至今任意一个月已缴纳完税凭证或税务机关开具的完税证明（任意税种）；依法免税的应提供相关文件证明；？（7）提供2021年1月至今任意一个月已缴纳的社会保障资金缴存单据或社保机构开具的社会保险参保缴费情况证明；依法不需要缴纳社会保障资金的应提供相关文件证明；（8）具备履行合同所必需的设备和专业技术能力的证明材料(由供应商根据项目需求提供说明材料或者承诺)；（9）参加政府采购活动前3年内，在经营活动中没有重大违法记录；？（10）根据《财政部关于在政府采购活动中查询及使用信用记录有关问题的通知》（财库〔2016〕125号）文件要求，采购代理机构将通过“中国执行信息公开网”（http://zxgk.court.gov.cn）、“信用中国”网站（www.creditchina.gov.cn）和“中国政府采购网”（www.ccgp.gov.cn）在磋商时查询供应商信用记录，被列入失信被执行人、税收违法黑名单、政府采购严重违法失信行为记录名单的单位将被拒绝参与本项目磋商。</p>
