<p>合同包1(口腔锥形束CT（CBCT)、数字化口腔全景X光线机)特定资格要求如下:</p> <div> <p>(1)投标供应商如为所投产品的生产企业，应依法取得《医疗器械生产许可证》或备案凭证；投标供应商如为所投产品的经销商，应依法取得《医疗器械经营许可证》或备案凭证。按国家规定执行；投标时须提供有效的《医疗器械生产许可证》或《医疗器械经营许可证》或备案证明复印件。</p> </div> <div> <p>(2)投标供应商在“信用中国网”（www.creditchina.gov.cn）、“中国政府采购网”（ www.ccgp.gov.cn）没有被列入失信被执行人、重大税收违法案件当事人名单、 政府采购、环境保护、知识产权等领域严重违法失信行为记录名单（以采购代理机构于投标截止日当天在两个网站的信用记录查询结果为准，如在上述网站查询结果
