0;">（1）、投标人应在中华人民共和国境内注册的企业法人、事业法人、其他组织或自然人，出具合法有效的营业执照或事业单位法人证书，自然人参与的提供其身份证明；（2）、投标人需具备医疗器械经营许可证或备案凭证，采购产品列入医疗器械管理的须提供相应的医疗器械注册证或备案凭证；（3）、投标人具有履行合同所必需的设备和专业技术能力的承诺函；（4）、投标人出具参加政府招标活动前3年内在经营活动中没有重大违法纪录的书面声明；（5）、单位负责人为同一人或者存在控股、管理关系的不同投标人，不得同时参加本项目采购活动；（6）、信用记录：投标人未被列入“信用中国”网站严重失信主体名单和税收违法黑名单；中国政府采购网政府采购严重违法失信行为记录名单中被财政部门禁止参加政府采购活动的投标人；（7）、法定代表人直接参加投标只须提供法定代表人身份证，非法定代表人投标，须出具法定代表人授权书及被授权人身份证；</p>
