0;">（1）供应商须具备独立承担民事责任能力的法人或其他组织，并出具合法有效的营业执照或事业单位法人证书等国家规定的相关证明（经营范围须符合本项目要求）；（2）财务状况报告：提供2021年度的财务审计报告；？成立时间至提交投标文件截止时间不足一年的可提供成立后其基本存款账户出具的资信证明；（3）税收缴纳证明：提供2021年7月1日至今已缴纳的至少连续3个月纳税证明或完税证明，依法免税的单位应提供相关证明材料；（4）社会保障资金缴纳证明：提供2021年7月1日至今已缴纳的至少连续3个月的社会保障资金缴存单据或社保机构开具的社会保险参保缴费情况证明，依法不需要缴纳社会保障资金的单位应提供相关证明材料；（5）书面声明：参加本次政府采购活动前三年内在经营活动中没有重大违纪，以及未被列入失信被执行人、重大税收违法案件当事人名单、政府采购严重违法失信行为记录名单的书面声明。本项目拒绝被列入失信被执行人、重大税收违法案件当事人名单、政府采购严重违法失信行为的供应商参与；？（6）提供具有履行合同所必需的设备和专业技术能力的承诺函；？（7）法定代表人授权书及被授权人身份证。（法定代表人直接投标提供法定代表人证明书及其身份证原件）；</p>
