</div> </div> </div> <p>3.本项目的特定资格要求：</p><div class="innercontent u-content"> <div> <p>合同包1(府谷县“十四五”产业招商引资规划编制项目)特定资格要求如下:</p> <div style="margin-left: 32px;"> <p class="u-content" style="text-indent: 0;">①供应商应须具有独立承担民事责任能力的法人或其他组织或自然人，须提供合法有效的统一社会信用代码的营业执照副本或事业单位法人证书等国家规定的相关证明原件，自然人参与的提供其有效身份证明原件；②拟派往本项目的项目负责人须具有经济等相关专业工程师及以上职称证书或注册咨询工程师，并提供社保经办机构出具的2022年5月、6月或7月份至少一个月的本企业社保缴纳证明材料（五险一金其中一项即可，应可查询）；③财务状况报告：提供2021年度的财务审计报告（成立时间不足一年的可提供成立后任意时段的资产负债表、利润表和现金流量表）或开标前近六个月内其基本存款账户开户银行出具的资信证明；④税收缴纳证明：提供2022年1月1日至今任意三个月的纳税证明或完税证明，依法免税的供应商应提供相关文件证明；⑤社会保障资金缴纳证明：提供2022年1月1日至今已缴存的至少三个月的社会保障资金缴存单据或社保机构开具的社会保险参保缴费情况证明，依法不需要缴纳社会保障资金的应提供相关证明材料；⑥信用要求：投标供应商、法定代表人不得为“信用中国”网（www.creditchina.gov.cn）中列入失信被执行人的供应商，投标供应商不得为“中国政府采购”网（www.ccgp.gov.cn）政府采购严重违法失信行为记录名单中被财政部门禁止参加政府采购活动的供应商。提供本项目发出公告之日至谈判文件递交截止时间前“信用中国”网站、“中国政府采购”网查询的相关信用记录完整截图（企业信用报告及完整截图），“信用中国”网站中投标供应商失信被执行人查询截图以“中国执行信息公开网”网站（http://zxgk.court.gov.cn/shixin/）中全国范围内查询为准。⑦供应商需提供具有履行合同所必需的设备和专业技术能力的承诺函；⑧书面声明：参加本次政府采购活动前三年内在经营活动中没有重大违法记录的书面声明函；⑨单位负责人为同一人或存在直接控股、管理关系的不同单位，不得同时参加本项目投标活动，提供《供应商企业关系关联承诺书》；⑩本项目专门面向中小企业采购，非中小企业单位（监狱企业、残疾人福利单位除外）不得参与投标。</p>
