0;">（1）提供有效合格的社会统一信用代码的营业执照，其他组织经营的须提供合法凭证，自然人的提供身份证明文件;（2）法定代表人授权委托书、被授权人身份证（法定代表人参加磋商时,只需提供法定代表人身份证）；（3）供应商具备建设行政主管部门核发的建筑工程施工总承包三级(含三级)或市政公用工程施工总承包三级(含三级)及以上资质，具有有效的企业安全生产许可证，并在人员、设备、资金等方面具备相应的施工能力；项目经理具备建筑工程或市政专业二级(含二级)以上注册建造师证书并具有安全生产考核合格B类证书且在有效期内，未担任其他在建工程；（4）财务状况报告：提供2021年度经审计的完整财务报告或开标日期前三个月内其基本存款账户开户银行出具的资信证明。（如提供资信证明，须同时提供基本存款账户开户许可证或基本账户信息表）；（5）税收缴纳证明：提供2022年1月至今已缴纳的至少一个月的纳税证明，依法免税的单位应提供相关证明材料；（6）社会保障资金缴纳证明：提供2022年1月至今已缴存的至少一个月的社会保障资金缴存单据或社保机构开具的社会保险参保缴费情况证明，依法不需要缴纳社会保障资金的单位应提供相关证明材料；（7）具备履行合同所必须的设备和专业技术能力的书面声明;（8）参加政府采购活动前三年内，在经营活动中没有重大违法记录的书面声明；（9）供应商未被列入信用中国网站(www.creditchina.gov.cn)“失信被执行人、重大税收违法失信主体”；不处于中国政府采购网(www.ccgp.gov.cn)“政府采购严重违法失信行为信息记录”中的禁止参加政府采购活动期间。</p>
