0;">（1）有效的主体资格证明：投标人为合法注册的法人（或其分支机构）、其他组织或自然人，具有独立承担民事责任的能力，提供具有统一社会信用代码证的营业执照（或事业法人证），非法人企业参照执行，投标人为自然人的提供身份证。（2）良好的商业信誉和健全的财务会计制度：提供？2020年度或2021年度的财务审计报告（至少包括资产负债表和利润表，成立时间至提交响应文件截止时间不足一年的可提供成立后任意时段的资产负债表）或开标前六个月内其基本存款账户开户银行出具的资信证明（附开户许可证或基本账户证明）。（3）具备履行合同所必需的设备和专业技术能力的证明材料（说明及承诺）。（4）依法缴纳税收和社会保障资金情况：提供2021年度至今已缴纳的至少一个月的纳税证明或完税证明及社会保障资金缴存单据或社保机构开具的社会保险参保缴费情况证明，依法免税或不需要缴纳社会保障资金的单位应提供相关证明材料。（5）参加政府采购活动前3年内在经营活动中没有重大违法记录的书面声明。（6）投标人应授权合法的人员参加投标全过程，其中法定代表人（或负责人）直接参加投标的，须提供法定代表人（或负责人）身份证，并与营业执照上信息一致。法定代表人（或负责人）授权代表参加投标的，须提供法定代表人（或负责人）授权书及被授权人身份证。（7）投标人不得为“信用中国”网站（www.creditchina.gov.cn）中列入失信被执行人和重大税收违法案件当事人名单的投标人，不得为中国政府采购网（www.ccgp.gov.cn）政府采购严重违法失信行为记录名单中被财政部门禁止参加政府采购活动的投标人。（8）本项目不接受联合体投标。（9）单位负责人为同一人或者存在直接控股、管理关系的不同投标人，不得同时参加本次采购活动。（10）为本采购项目提供整体设计、规范编制或者项目管理、监理、检测等服务的投标人，不得再参加本采购项目的其他采购活动。（11）本项目接受进口产品投标，所投产品为进口产品的，须提供所投产品生产厂家授权书。</p>
