0;">1.有效的企业法人营业执照、税务登记证、组织机构代码证或三证合一营业执照；2.法定代表人参加磋商的，须提供本人身份证原件并提供本人身份证复印件（附到资格证明文件中）；法定代表人授权他人参加磋商的，须提供法定代表人委托授权书并提供被授权代表的身份证原件；3.提供具有履行合同所必需的设备和专业技术能力的承诺函；4.信用要求：投标人参加本次招标近三年内，在经营活动中没有重大违法记录，在参加政府采购活动中未被监督管理机构列入“不良行为记录名单”,投标人需提供自述材料及供应商未被列入“信用中国”网(www.creditchina.gov.cn)？“信用服务”中“失信被执行人、重大税收违法案件当事人名单、政府采购严重违法失信名单”？的查询记录截图、中国政府采购网(？www.ccgp.gov.cn)“政府采购严重违法失信行为信息记录”的查询记录截图（提供查询结果网页截图并加盖投标人公章）。如自述材料与实际不符，将按不合格处理，取消投标资格；5.提供2021年度经审计的财务审计报告（至少包括审计报告、资产负债表和利润表，成立时间至提交投标文件截止时间不足一年的可提供成立后任意时段的资产负债表）；或其基本存款账户开户银行出具的资信证明及基本存款账户开户许可证；6.本项目不接受任何形式的联合体磋商。</p>
