（3）相关规划成果、相应批复，以及规划成果的主要结论等；规划项目建设情况； （4）国民经济统计数据：流域经济社会统计数据，包括人口、国民生产总值、粮食产量、畜禽养殖、土地利用、废污水及主要污染物排放量等统计数据，作为河流生态损害问题分析的依据； （5）基础图件：河流流域水系图、涉水工程现状图、行政区划图、土壤类型图、植被覆盖图等基础信息图件等，用于编制河流流域基础信息图册。 （6）指标体系赋分计算所需基础数据资料，由采购人协调相关主管部门提供。 2.评价分区及调查监测 根据黑石河、干溪河流调查的情况及相关主管部门提供的资料，对黑石河、干溪河水文特征、河床及河滨带形态、水质状况、水生生物特征以及流域经济社会发展特征，沿河流纵向将黑石河、干溪河分为若干评估河段，评估河段划分应满足《指南》要求；通过对各个河段开展健康评价调查与专项监测工作，综合得出黑石河、干溪河整体评价结果。 3.系统整理分析各评价指标调查监测数据，根据“指南”要求计算黑石河、干溪河健康评价指标赋分，评价黑石河、干溪河健康状况，分析河流健康问题并提出对应保护对策，编制黑石河、干溪河河流健康评价报告并组织行业专家对黑石河、干溪河健康评价报告进行技术审查，根据专家意见修改完善报告，形成最终版黑石河、干溪河河流健康评价报告。 4.评价报告内容应包括：崇州市黑石河、干溪河河流基本情况、河流健康评价方案、河流健康调查监测、河流健康评价结果、河流健康问题分析与保护对策、河流健康评价专题附图（包括河流水系图、土壤类型图、植被覆盖图、河流健康调查监测方案专题图等）、河流健康评价专题附表（包括评价河段分区、监测点位、调查表、生物物种名录及其照片等）。 （三）成果提交要求
