<p>1）具有独立承担民事责任的能力：提供在中华人民共和国境内注册的法人或其他组织的营业执照或事业单位法人证书或社会团体法人登记证书复印件，如投标人为自然人的提供自然人身份证明复印件；如国家另有规定的，则从其规定，以上内容须加盖投标人公章；分支机构投标，须取得具有法人资格的总公司（总所）出具给分支机构的授权书，并提供总公司（总所）和分支机构的营业执照（执业许可证）复印件加盖投标人公章。已由总公司（总所）授权的，总公司（总所）取得的相关资质证书对分支机构有效，法律法规或者行业另有规定的除外。</p> <p>2）具有良好的商业信誉和健全的财务会计制度：提供投标截止日前三年中任意一年度或任意一个月财务状况报告（包括但不限于资产负债表、利润表），或投标截止日前6个月内基本开户行出具的资信证明。</p> <p>3）有依法缴纳税收和社会保障资金的良好记录：提供投标截止日前6个月内任意1个月依法缴纳税收和社会保障资金的相关材料。如依法免税或不需要缴纳社会保障资金的，提供相应证明材料。</p> <p>4）履行合同所必需的设备和专业技术能力：投标人须提供资格声明函，格式自拟。</p> <p>5）参加采购活动前3年内，在经营活动中没有重大违法记录：投标人须提供资格声明函，格式自拟。</p> <p></p> <p>2.落实政府采购政策需满足的资格要求： <span class="u-content">无。</span> </p> <p>3.本项目的特定资格要求：</p><div <p>(1)投标人未被列入“信用中国”网站（www.creditchina.gov.cn）以下任意记录名单之一：①失信被执行人；②重大税收违法失信主体；③政府采购严重违法失信行为记录名单。同时，不处于中国政府采购网（www.ccgp.gov.cn）“政府采购严重违法失信行为信息记录”中的禁止参加政府采购活动期间。工商注册所在地在珠海市的投标人还须同时提供“信用中国（广东珠海）”网站（http：//credit.zhuhai.gov.cn/）的信用记录查询结果。（说明：①投标人的投标文件可不提供本项证明文件，由采购代理机构于投标截止当日在“信用中国”网站（www.creditchina.gov.cn）及中国政府采购网（www.ccgp.gov.cn）查询结果为准，如在上述网站查询结果均显示没有相关记录，视为不存在上述不良信用记录。②采购代理机构同时对信用信息查询记录和证据截图或下载存档。③投标人为分支机构的，同时对该分支机构所属总公司（总所）进行信用记录查询，该分支机构所属总公司（总所）存在不良信用记录的，视同投标人存在不良信用记录。）。</p>
