font-size: 16pt">其他要求</span> </td> <td colspan="5" style="width:80%;"> <span lang="EN-US" style="font-family: 黑体; font-size: 16pt">1、本工程评标办法采用“报价承诺法”，实行“不见面开标项目”方式，采用“不见面开标”系统开标，投标企业不需要到场参加开标会，具体操作详见招标文件的其他材料里的附件“不见面开标”注意事项； 2、本工程招标控制价和要约价详见招标文件，各投标人必须响应要约价参与本次投标，否则作废标处理；3、本工程投标人必备的资格条件（资格审查应提供的证件材料)： ①企业资质证书（可查询二维码的资质证书可提供清晰的复印件并加盖公章）、营业执照（加盖章企业公章的扫描件或复印件）、安全生产许可证（加盖章企业公章的扫描件或复印件）；②法定代表人证书或委托代理人授权书，且为本单位正式职工，提供当地社保部门有效的证明材料（不含开标当月，连续6个月，因工作单位变更不足6个月的以证书的变更时间为准）、劳动聘用合同并加盖单位公章并附本人身份证；③市政公用工程注册建造师二级及以上证书及B类安全生产考核合格证，并附本人身份证；④拟派[施工员、质检员（质量员）]市政公用专业、材料员的岗位证书（或资格证书）和专职安全员的安全生产考核合格证书，并需提供以上人员在开标当月之前（不含开标当月，连续6个月）社保部门出具的有效社保证明材料（因工作单位变更不足6个月的以证书的变更时间为准）及身份证复印件加盖公章；⑤外省来赣的投标人应根据《关于房屋市政工程施工电子招投标活动中省公共资源交易平台对接共享江西住建云平台信息数据的通知》（赣建城[2021]19及号）文件，以及2021年8月3日江西公共资源交易网发布的《关于江西省公共资源交易平台对接江西住建云平台信息数据紧急通知》，本项目外省来赣的施工单位需提供在江西住建云平台（或江西省公共资源交易平台）省外建设工程企业进赣信息登记管理系统上办理好有效的企业信息登记，且提供企业信息的网上查询结果截图加盖本单位公章扫描件；无需办理本项目单项工程投标登记，无需提供单项工程投标登记查询的相关材料；4、近6个月内（截止到开标前一日）被江西省建设行政主管部门记录不良行为（符合全国建筑市场各方主体不良行为记录认定标准）的不得参加本工程投标；被列入赣州市政府投资工程限制投标承包商（黑名单）且尚处在限制投标期限内的投标企业，不得参加本次投标；投标人36个月内在中华人民共和国境内没有发生被住房和城乡建设行政主管部门通报的骗取中标、串通投标、重大工程质量、安全生产事故问题；投标人或投标人法人代表及拟派项目负责人36个月内无人民检察院行贿犯罪档案网络查询系统中可以查询到的行贿记录；企业近3个月内没有因拖欠工人工资被招标项目所在地省、市、县（市、区）建设行政主管部门通报批评的。5、本工程不接受联合体投标； 6、本工程项目管理班子人员考勤及人员变更严格执行赣市建字〔2018〕115号《关于进一步完善我市房屋建筑和市政基础设施工程施工、监理单位项目班子管理人员电子考勤的通知》，被限制投标资格并正处在限制期限内的企业不得参加本工程的投标，中标的中标无效；7、根据最高院等九部门“关于在招标投标活动中对失信被执行人实施联合惩戒的通知” （法〔2016〕285号文）及“江西省房屋建筑和市政基础设施工程施工招投标资格审查办法”（赣建招（2017）1号文） 第二十一条（五）出现下列情形的投标人不得参与投标：投标人或投标人法人代表及拟派项目负责人被列入“信用中国”网站（www.creditchina.gov.cn）失信被执行人、重大税收违法案件当事人名单、政府采购严重违法失信名单（招标公告发布前从最高人民法院失信被执行人信息库中删除的除外）；8、积极开展“扫黑除恶”专项斗争，严厉打击工程招投标领域雇黑拥黑、恶意竞标、暴力围标、强揽工程等黑恶势力。崇义县城镇发展服务中心举报电话：0797-7******；9、其它未尽事宜要求详见招标文件，本招标公告与招标文件有不一致处以招标文件约定的内容为准。</span> </td> </tr> <tr> <td align="center" colspan="6"> <span style="font-family: 黑体;
