style="text-indent: 0;">（1）供应商须具有企业法人或其他组织有效的三证合一的营业执照，自然人应提供身份证明；（2）供应商须具备建设行政主管部门颁发的工程设计专业资质市政行业道路工程丙级及以上资质；（3）拟派项目负责人须具备相关专业中级及以上职称；（4）提交近一年内任意3个月完税凭证或税务机关开具的完税证明，依法免税的应提供相关文件证明；（5）提交近一年内任意3个月的社会保障资金缴存单据或社保机构开具的社会保险参保缴费情况证明，依法不需要缴纳社会保障资金的应提供相关文件证明；（6）提供履行合同所必须的设备所有权证明、和专业技术能力证明材料；（7）供应商须提供2020或2021年度经审计的财务报告或近六个月内银行出具的资信证明；（8）出具参加政府采购活动前3年内在经营活动中没有重大违法纪录的书面声明；（9）供应商不得为“信用中国”网站中列入失信被执行人（www.creditchina.gov.cn）和重大税收违法案件当事人名单的供应商，不得为中国政府采购网（www.ccgp.gov.cn）政府采购严重违法失信行为记录名单中被财政部门禁止参加政府采购活动的供应商；（10）本项目不接受联合体磋商。</p>
