②财务状况报告：提供2021年度经审计的财务会计报告（含四表一注，成立时间至提交投标文件截止时间不足一年的可提供成立后任意时段的资产负债表），或其开标前三个月内基本开户银行出具的资信证明，或信用担保机构出具的投标担保函（以上三种形式的资料提供任何一种即可）； ③社保缴纳证明：提供递交投标文件截止之日前一年内任意一个月的社会保障资金缴存单据或社保机构开具的社会保险参保缴费情况证明。依法不需要缴纳社会保障资金的供应商应提供相关证明文件； ④税收缴纳证明：提供递交投标文件截止之日前一年内任意一个月的依法缴纳税收的相关凭据（时间以税款所属日期为准）。依法免税或无须缴纳税收的供应商，应提供相应证明文件；⑤提供具有履行本合同所必需的设备和专业技术能力的承诺；⑥提供参加政府采购活动前三年内，在经营活动中没有重大违法记录的招标响应声明书；⑦单位负责人为同一人或者存在直接控股、管理关系的不同供应商，不得参加同一合同项下的政府采购活动；⑧供应商不得为“信用中国”网站（www.creditchina.gov.cn）中列入失信被执行人和重大税收违法案件当事人名单的供应商，不得为中国政府采购网（www.ccgp.gov.cn）政府采购严重违法失信行为记录名单中被财政部门禁止参加政府采购活动的供应商；⑨本次采购不接受联合体投标。</p>
